Student Medication Policy

Please be aware that all medication must be brought to the office when the student enters the building, including any non-prescribed medication. Non-prescribed AND prescription medication must accompany a Chelsea School District Medication Prescriber/Parent Authorization form available below or in the office. Prescription medication must be authorized by a physician either with a signature on this form or with a current prescription bottle. Thank you for your support!
