BUS STOP REVIEW REQUEST
This form is used to identify necessary information in order to review whether a bus stop needs to be changed, added or deleted. Please complete the attached form for Bus Stop Review consideration. Written request must be submitted to the Transportation Office at least ten (10) school days prior to the desired date for the change to allow time for the proposed change to be reviewed.
